TL;DR: Key Takeaway

verdict

Rock The Rankings ranks first for B2B health tech SaaS teams that want SEO and AI search run as one program and measured in demos. Digital Elevator is the strongest alternative for biotech and medtech brands, with published managed SEO from $5,000/mo. Check who an agency actually serves first, because most healthcare SEO agencies market clinics, not software.

Pros
  • One program across Google and the answer engines, not an SEO retainer with GEO bolted on
  • A three-scenario forecast published before you sign, and graded monthly after
  • Two-signal attribution that still works when assistants strip the referrer
  • You work with the founder from first call through monthly report
Cons
  • Search only, SaaS only. If you want one vendor for paid, lifecycle, social, and brand, we are not it.
  • There is a 90-day initial commitment, because this compounds.

How we scored

The criteria, weighted for pipeline.

We reviewed public case studies, client rosters, pricing pages, and third-party reviews. One question outranks the rest: does the agency connect search activity to pipeline you can audit, or does it report traffic and hope you are impressed.

SEO & AI search

Rock The Rankings is the SaaS SEO agency built around one uncomfortable finding: in a single funnel of 8,307 contacts, the CRM logged 11 AI-attributed leads while 809 buyers named an AI assistant on the intake form. A 73× gap, invisible to every dashboard on the market. That audit became our attribution method, and the method became the program.

We are a compact senior team, one discipline, one customer type. You work one-on-one with the founder from first call to the monthly report, every engagement opens with a three-scenario forecast on your funnel data, and we get graded against it in public. The most recent 90-Day AI Search Program forecast 10 demos and delivered 59.

How we're different than the other agencies on this list

1. We only do SEO (and AI search), nothing else

Every agency pitch deck says "full service". In practice, full service means your budget subsidizes their learning curve in four other channels.

We run one discipline: SEO and LLM search for B2B SaaS. Content strategy, technical, links, and the AI search layer on top of all three. That is the entire menu, and it has been since 2019.

We're not the right fit if you want one agency to run SEO plus paid, social and email.

The depth shows up where it counts: taking Keragon from DR 0 to 68 without buying a single link is not a generalist outcome. It is what a team that does exactly one thing, thousands of reps deep, produces on schedule.

2. We only work with SaaS/Tech, no one else

Nearly every SEO agency "works with SaaS". Most also work with ecommerce, DTC, local services and whoever signed last quarter. Every new vertical resets their playbook to zero, and you pay for the reset. Ours never resets. Same buyer, same funnel math, same page types, every engagement since 2019.

That focus buys you three things on day one:

  • The channel map already exists. We know the publications, review sites and sources that shape your category, because we have already put clients in them.
  • Benchmarks, not guesses. We know what organic growth, content velocity and demo volume look like for a SaaS company at your stage, so we can tell you whether a result is good instead of just reporting that it happened.
  • The money pages are known quantities. Alternatives pages, comparisons, category listicles: we know which page types convert SaaS buyers and which just decorate a traffic chart.

We only take SaaS/tech companies with product-market fit, and we forecast the engagement before we accept it.

The practical difference: you stop spending the first six months of the retainer educating your agency about your own market.

3. No junior account managers. Work 1-on-1 with our founder.

You know the agency bait-and-switch. Senior strategists run the sales calls, then the contract is signed and your account lands with a coordinator two years out of school who has no vested interest in whether you renew. That handoff is structurally impossible here:

  • You work one-on-one with our founder from kickoff to the last call. Not "founder oversight". The founder, doing the work.
  • We cap the number of clients we take each quarter, so nobody's engagement is subsidizing a churn-and-burn roster.
  • The team is compact and senior, with an average tenure of 3.9 years. The person who starts your engagement finishes it.

The receipts match the claim: a 5.0 rating across 13 Clutch reviews, and the reviews name Justin, because clients actually work with Justin.

4. We forecast and expectation set from the get-go. No guesswork.

The standard agency proposal is a canned deck, a price, and a promise to "move the needle". Nothing in it can be graded later, which is exactly the point of writing it that way.

We do the opposite. Before kickoff, we model three scenarios on your funnel data: Conservative, Realistic and Optimistic, each with the content plan, link velocity and pipeline math shown in full. If the model cannot clear a 4× return, we say so on the call and do not take the engagement. Then we grade ourselves against the model in public. WaitWell's program cleared its entire four-month Realistic forecast inside month one and finished at 2.9× Realistic and 7.9× Conservative.

And we count results the hard way: a lead only counts when two independent sources agree on where it came from. When we audited one funnel's "AI-driven" pipeline, 809 self-reported AI leads reduced to 11 the CRM could actually attribute, a 73× gap. We report the 11. Most agencies would have invoiced you for the 809.

You see the forecast before you sign, and you grade us against it every month after.

5. The four SaaS SEO problems we are hired to fix

Almost every SaaS team that calls us is stuck on one of four things. Here is each problem, what actually causes it, and the exact artifact we ship against it.

Problem 01

"We rank, and pipeline didn't move."

The cause: the keyword set was built for volume. You own definitional terms your buyers read once and never convert on, while competitors own the comparison and alternative terms buyers read with a credit card open.

What we ship: a BOFU-first content plan. Alternatives pages, comparisons and category listicles ship before a single blog post, because those are the pages buyers and AI assistants actually decide from. Rasa's first batch targeted shortlist queries only; demo attribution moved with traffic instead of lagging it by two quarters.

Head-to-head comparisons5BOFUW5–9
TOFU blog volume—TOFUNot first

The first pages are money pages. TOFU comes after the revenue layer exists, not before.

Problem 02

"Traffic grew. Demos didn't."

The cause: the agency was graded on sessions, so it optimized for sessions. Rankings landed on terms your buyers never buy from, and the report celebrated a number your CFO cannot spend.

What we ship: pages mapped to shortlist queries, then reporting that grades traffic by whether demand moved with it. Rasa is the reference case: because the first batch targeted only the queries buyers shortlist from, demo attribution moved in the same quarter as traffic instead of lagging it by two.

Rasa · sessions to the shortlist pages, indexed to the January baselineReceipt

The quarter closed 804% above the prior quarter, corroborated across two independent analytics properties within 2%. Demos attributed to search and AI grew 2.3× in the same quarter, which is the point: traffic that buyers shortlist from converts in-quarter, not two quarters later.

Problem 03

"ChatGPT recommends our competitors, not us."

The cause: the models answer from third-party sources, listicles, review sites and industry roundups, and your brand is not in them. If the sources the models quote never name you, no amount of on-site content fixes it.

What we ship: a citation baseline across five engines, then targeted pages, placements and brand mentions on the exact sources the models are already quoting for your category. Every blank cell becomes a build target with an owner and a date.

From a live kickoff baseline: named in 4 of 20 buying prompts. Lendbuzz ran this play to 21 of 23 tracked themes, 2.3× ahead of the nearest competitor. Pages and mentions are how you fill the rest.

Problem 04

"Half our pipeline shows up as Direct Traffic."

The cause: AI assistants strip the referrer, so a buyer who spent a week comparing vendors inside an assistant arrives looking like they typed the domain from memory. The CRM files them as Direct, and Direct is not a channel. It is the system saying it does not know. WaitWell's classifier filed 42% of demo bookers that way.

What we ship: two-signal attribution. Every contact is read on two signals, the click record and the buyer's own answer on the form, resolved by a written five-case matrix with contacts carrying neither signal reported openly as Dark, never reallocated. Then the recovered channel gets graded against a forecast published before kickoff, so the number is both real and accountable.

When to pick someone else on this list

We are the wrong choice for plenty of teams, and pretending otherwise wastes your call and ours. Honest routing:

  • You sell to biotech, medtech, or lab buyers and want AI visibility sprints → Digital Elevator (#2), managed SEO from $5,000.
  • You want SEO on a published budget, with month-to-month terms → RevenueZen (#4) or GrowPad (#3), both from $3,000.
  • You need medically reviewed content at scale and have the budget → Siege Media (#5).
  • You want a full health IT marketing team, not search alone → Clarity Quest Marketing (#7).
  • You are a provider group selling appointments, not software → Cardinal Digital Marketing (#10).
  • You are pre-product-market fit → nobody on this list yet. SEO compounds on top of a working funnel. Fix the funnel first, then come back.

If none of those describe you and organic pipeline is the project, book the call. You will get a forecast either way.

Mini case studies

Rasa: AI search became a demand channel in one quarter

Rasa builds the conversational AI platform enterprise teams use to design, deploy and govern their own assistants. Two problems compounded: the commercial page set buyers shortlist from was losing sessions month after month, and the channel that was working was invisible, because AI engines strip the referrer the CRM classifier depends on.

We shipped bottom-funnel pages against shortlist queries only, built them to be quoted and not just ranked, then read every contact on two signals so the demand could be seen and funded.

One quarter from launch: organic sessions to the shortlist pages up 9× (+804%), attributed demo requests up 2.3× quarter over quarter, and Rasa was the only tracked brand in its category to gain non-branded AI presence while every rival lost ground. The corroborated demand figure ran 12.5× what the analytics classifier credited, reaching 23× in the peak month.

WaitWell: AI search booking demos at 2.9× forecast in four months

WaitWell builds queue management and appointment scheduling software for healthcare, government, education and service counters. The CRM filed 42% of demo bookers as Direct Traffic, the bucket that means the system does not know, so a channel already producing customers looked like it barely existed.

We published a three-scenario forecast before a single page shipped, built best-of and alternatives pages against the shortlist queries, and read every contact on two independent signals.

Four months in: demos from organic and AI search at 2.9× the Realistic forecast and 7.9× the Conservative case, new customers at 3.3× and 8.9×, bottom-funnel Google clicks up 79%, and the money term moved from page two to position 3.7.

Lendbuzz: organic conversions up 200% on a page set that didn't exist a year ago

Lendbuzz underwrites car loans using income, education and employment data rather than a US credit score alone. Buyers describe a situation, not a product, and increasingly they describe it to an AI assistant. Two quarters before this window, the pages that now carry the growth did not exist.

We built a content cluster around the circumstances buyers actually describe, widened onto the highest-value commercial terms as the business repositioned, and measured only what the program is responsible for: key events segmented to organic, on pages we built, homepage excluded.

Result: the two conversion events entering through the built pages up 200% and 276% year over year, organic sessions to those pages up 69×, and Lendbuzz named in 21 of 23 tracked AI answer themes, 2.3× ahead of the nearest competitor.

SEO & AI search

Digital Elevator is a West Palm Beach SEO and AI search agency that has repositioned around AI visibility, selling 90-day sprints alongside a managed AEO and SEO retainer. Most of its published work is in biotech, life science instrumentation, medtech, and pharma, with clients such as McKesson, Halo Labs, BMG LABTECH, and Champions Oncology. It suits health tech marketers who sell into labs, clinicians, or procurement and want to show up in ChatGPT and Google AI Overviews, though health IT and healthcare SaaS work is thinner in its portfolio.

Tradeoff
The site itself says compliance-heavy work is generally not the best fit for its program model, so regulated health tech teams with formal medical or legal review should plan to run that sign-off in house. The team is also small, at 2 to 9 people on Clutch.

GrowPad is a SaaS SEO and AI search agency based in Dnipro and Alicante with a dedicated healthcare and health tech practice. Its longest health tech engagement is Kodjin, Edenlab's FHIR server, where a new domain grew from DR 14 to 52 and produced 5+ SQLs per month while competing with Microsoft and InterSystems. It also lists Edenlab's healthcare analytics work and TATEEDA GLOBAL, a healthcare software developer, which makes it a credible pick for interoperability, health data, and health IT vendors with long sales cycles.

Tradeoff
GrowPad does not describe any HIPAA, FDA, or medical review process, so regulated content still needs an internal reviewer. Its healthcare page also says it is not a fit for pre-product companies or teams expecting results within 60 days.

RevenueZen is a Portland B2B organic growth agency that runs SEO, AI search, content, and LinkedIn programs for software companies, with clients like Expensify, Lightyear, and Bloomfire. It has a dedicated healthcare and health tech page and has worked with Carevoyance, a sales enablement platform for medtech, and MedCompli, a healthcare compliance SaaS. Health tech buyers get B2B SaaS pipeline experience and flexible terms, but should expect to supply their own clinical and regulatory review.

Tradeoff
Healthcare is one vertical among many B2B tech niches, with only a couple of documented health tech engagements and no medical review process described, so clinical accuracy checks fall to the client.

Content, digital PR & AI search

Siege Media is an Austin-founded content, SEO, and digital PR agency that now sells a full Generative Engine Optimization program aimed at ChatGPT, Claude, and Google AI Overviews. Its health practice pairs in-house strategists with a vetted network of more than 4,000 credentialed experts so YMYL content meets medical accuracy and compliance standards. Health tech clients include teletherapy platform Grow Therapy and Healthline, which makes it a fit for digital health companies that compete on trusted educational content.

Tradeoff
Siege is a premium content and PR shop whose health wins lean toward consumer-facing brands, so an early-stage B2B health SaaS team with a small budget may find the entry price high and the playbook less tuned to buying committees.

Digital Authority Partners is a Chicago-based digital agency founded in 2016 with a heavy healthcare client base. Its health tech work includes SEO for imaware's at-home testing platform, a revenue lift for the Epocrates physician app from athenahealth, Omron Healthcare, and environmental monitoring company Dickson. It suits health tech companies that want SEO bundled with site rebuilds, migrations, and product marketing rather than a pure content program.

Tradeoff
DAP is a full-service shop spanning web development, apps, paid media, and SEO, so a startup that only wants focused SEO and AI search content may pay for breadth it does not need, and pricing is quote-only.

Full-service B2B healthcare marketing

Clarity Quest is a full-service B2B marketing agency for healthcare technology, medical device, and biotech companies, operating since 2001 and now part of Supreme Group. It covers planning, branding, ABM, PR, paid media, content, email, and SEO, and can staff a complete outsourced marketing team for companies up to about $80 million in revenue. Health tech software buyers get deep category familiarity, but SEO sits inside a broad service mix rather than leading it.

Tradeoff
SEO is one line item inside a full-service marketing menu, with no published AI search offering, and the $50,000 project minimum prices out early startups.

Thought leadership SEO & AI search

First Page Sage is a thought leadership SEO agency founded in 2009 by Evan Bailyn that now sells SEO and Generative Engine Optimization together. Its medtech practice has produced clinical trial management software comparison series, healthcare CRM white papers, and HIPAA compliance resource centers for B2B health tech buyers. Named health clients include Biovia, Altoida, Intuity Medical, and Dignity Health, which fits companies selling software or devices into health systems.

Tradeoff
First Page Sage runs long, writer-heavy thought leadership programs that third parties put at roughly $8,000 per month and up, which is a lot for a seed-stage health startup that needs fast, narrow wins.

Healthcare content & SEO strategy

Aha Media Group is a healthcare-only content agency founded by Ahava Leibtag that provides content strategy, writing, and content operations. Most of its named clients are health systems such as Penn Medicine, Cleveland Clinic, and Stanford Health Care, though it also serves health tech, medtech, and B2B healthcare companies, including NeuroPace. It suits health tech buyers who need accurate clinical content and an SEO and AI search content strategy, more than a full SEO program.

Tradeoff
Aha is a content strategy and writing shop first, so technical SEO, link building, and pipeline attribution need another partner, and most of its proof comes from hospitals rather than software vendors.

Healthcare paid media & SEO

Cardinal is an Atlanta-based healthcare performance marketing agency, founded in 2009 and acquired by Power Digital in January 2026 as its dedicated healthcare division. It runs paid media, SEO and AI search optimization, analytics, and websites for multi-location provider groups such as ATI Physical Therapy, Duly Health and Care, and Peak Dental. Health tech software vendors are not its core audience, so it fits clinics and provider platforms better than B2B SaaS.

Tradeoff
Cardinal is built around patient acquisition for provider groups, so B2B health tech companies selling software to those providers will find little matching playbook or proof.

How to choose

Six questions to ask before you sign.

1

Which health tech companies have you grown, and what did they sell?

Ask for named B2B health tech clients, not hospitals or clinics. Patient-acquisition SEO and selling software to health systems run on different keywords, buyers, and sales cycles.
1

Who reviews clinical and regulatory claims before a page ships?

Ask whether a clinician or regulatory reviewer signs off, and how FDA-sensitive claims are handled. A content team without a review step will either water the copy down or publish claims your legal team has to pull.
1

How do you track conversions without exposing health data?

Ask how forms, pixels, and analytics are set up on pages a patient or clinician might use. Tracking on health pages has drawn regulator and litigation attention, so a careless setup is a compliance problem as well as a data problem.
1

How do you earn AI search citations on health topics?

Ask which prompts they track and in which assistants. AI answers on health topics lean on authoritative sources, so ask how they get your brand into the publications and directories those assistants already quote.
1

How do you connect a keyword to MRR?

Ask for the chain: which page, which term, which conversion event, and what the reporting shows two quarters later. Agencies that cannot draw that line will lead with traffic and rankings because those are the only numbers they hold.
1

What would you not do for us?

The most useful answer on any of these calls. An agency with a real point of view will tell you which channel or tactic they would decline, and why. One that says yes to everything is scoping to your budget rather than your problem.

FAQs

Common questions about hiring an agency

If yours is not here, bring it to the call. Same unpolished answer either way.

1

What is the best SEO agency for health tech companies?

+

For B2B health tech SaaS teams that want SEO and AI search tied to demos, we would argue for us, and we published the criteria so you can check the reasoning. Digital Elevator for biotech and medtech brands, GrowPad or RevenueZen on a published budget, and Siege Media when content needs medical expert review.

1

How much does health tech SEO cost?

+

Published floors on this list run from $3,000 a month at GrowPad and RevenueZen to $5,000 at Digital Elevator, and ours starts at $4,050. Siege Media publishes digital PR from about $12,000 a month, Clarity Quest Marketing lists a $50,000 project minimum on Clutch, and four of the ten price on request.

1

What is the difference between healthcare SEO and health tech SEO?

+

Healthcare SEO usually means patient acquisition for clinics and hospitals: local search, reviews, and appointment pages. Health tech SEO sells software to health systems, payers, and life sciences teams, with longer sales cycles and buying committees. Cardinal Digital Marketing and Aha Media Group lean toward the first, and most of the rest serve the second.

1

Does health tech content need medical review?

+

Any page that makes clinical or regulatory claims should be reviewed by someone qualified before it ships, and Google weighs expertise heavily on health topics. Siege Media runs an expert reviewer network and Aha Media Group writes clinical content. Ask every other agency here who signs off before publish.

1

Can SEO and analytics tracking create HIPAA risk?

+

Yes. Tracking pixels and analytics on pages where patients or clinicians enter health information have drawn regulator and litigation attention. Ask any agency how it configures forms, pixels, and analytics before it touches your site, and bring your compliance lead into that conversation.

1

How is this list scored, and did you rank yourself first?

+

Seven weighted criteria, fixed before scoring and re-scored quarterly, with health tech focus weighted highest. Yes, we ranked ourselves first. Every entry carries the same fields, the same criteria, and a tradeoff line, including ours, and where a figure is not public we say so rather than filling the gap.
